Choose to receive either a sample of Parent’s Choice Milk DHA & ARA Formula or Parent’s Choice Gentle Formula for fussiness or gas. The website form asks for your name, email address, mailing address, and phone number. It also requires you to add you baby’s birth or due date, list which formula you currently use, and record where you heard of Parent’s Choice.
The sample will take 3 – 4 weeks to arrive.
